The ISBAR Clinical Training System is an AI-powered platform for practising inter-hospital referral handovers. It is built for NCHDs, medical students, and nurses to rehearse structured clinical communication, receive instant AI feedback, and track their skills over time — safely, before they ever pick up the phone on a real ward.

The problem
Handover is where clinical information is most often lost. A rushed or unstructured referral call can omit the one detail that changes management. ISBAR (Identify · Situation · Background · Assessment · Recommendation) is the accepted framework — but trainees rarely get to practise it with feedback. This system turns that practice into a measurable, repeatable, and even enjoyable habit.
What it does
Four capabilities work together:
- Voice-first practice — trainees record a real referral call. Whisper AI transcribes the audio, then analyses the ISBAR structure automatically.
- Instant AI feedback — completeness scores per ISBAR component, identified gaps, and coaching notes after every handover.
- RPG progression — levels, badges, streaks, and a leaderboard turn repeated practice into progress you can see.
- Supervisor dashboard — educators get team analytics, audit tools, and performance insights.
The platform is HSE-aligned, GDPR-compliant, piloted at Midland Regional Hospital Tullamore, and free for trainees.
The trainee portal
After logging in, each clinician gets an anonymised identity — a pseudonym and avatar (e.g. NCHD-Epsilon-98) — so performance data never exposes who they are. The RPG dashboard shows their grade, level, and earned credentials.

Practice mode
Trainees choose a skill stream (General Internal Medicine, Surgical, Gynaecology & Obstetrics, Paediatrics), a subspecialty, a case level, and a specific clinical scenario. Each case loads a realistic chart to hand over from — for example, new-onset AF, rate 110, haemodynamically stable.

They then open the dictation console, pick the receiving specialty, review a pre-consult checklist, and dictate their handover. The app listens, transcribes, and returns structured feedback against the case.

Test mode
Once confident, trainees enter certification test mode. Here no scenario is loaded — they must formulate and deliver a complete ISBAR handover from clinical memory. Performance is graded under specialist rigour and feeds into their Star Standard certification.

The analytics hub
Every practice and test run flows into the NCHD Analytics Hub. An anonymised weekly leaderboard ranks clinicians by stars, level, and handover-case completeness.

Each trainee also gets an individual drill-down: current level, stars, daily streak, quality index, and rolling completeness rates across the five ISBAR components.

Progression
Skill maps to a clear grade roadmap — from Intern through SHO, Registrar, Senior Registrar, and finally Consultant — each unlocked by levels and XP earned through consistent, high-quality handovers.
